MyPractice Primary Care Plus (with Screening Activity Report data)

The MyPractice Primary Care Plus (with Screening Activity Report data) streamlines access to the Screening Activity Report (SAR) and MyPractice Primary Care Physician Report via a single report available on the eReport platform, through ONE ID registration.

This version of the MyPractice Primary Care Plus (with Screening Activity Report data) is the first step in a multi-year strategy to develop a tailored, consolidated and comprehensive report focused on quality improvement in primary care.   • Available to primary care physicians who practice as part of a patient enrolment model (PEM) and their delegates
  • Includes practice, regional and provincial-level data to allow physicians to compare and identify trends in screening activity in relation to peers in PEM practices
  • Provides patient-level screening data for breast, cervical, and colorectal cancers to help physicians improve their cancer screening rates and appropriate follow-up
  • Interactive platform, allowing physicians to find specific cancer screening information for each eligible patient
  • Updated on the 10th calendar day of each month to provide timely screening data
  • Available to non-salaried primary care physicians who work in a comprehensive family medicine setting and their delegates
  • Includes practice, regional and provincial-level data to allow physicians to compare and identify trends in care, in relation to their peers
  • Provides data on opioid and antibiotic prescribing trends, cancer screening, diabetes management, and use of other health services like emergency departments
  • Includes quality improvement suggestions, resources and tools so physicians can spend less time looking for solutions and more time helping patients
  • Updated bi-annually in June and December

Sioux Lookout and Zone (SLZ) Screening Activity Report (SAR)

This report is an online resource available to:

  • Non-patient enrollment model or alternatively funded model of primary care for the Sioux Lookout Municipality and Sioux Lookout Zone (27 First Nations communities)
  • The Hugh Allen Clinic Family Health Group
  • 27 First Nations communities serviced by the Sioux Lookout Regional Physicians Services Inc.

It provides screening data for breast, cervical, and colorectal cancers to help physicians and nurses improve their cancer screening rates and appropriate follow-up. The report platform is interactive, allowing report users to quickly find specific cancer screening information for each patient by community.

ONE ID is an identity and access management service that is home to a variety of reporting resources for physicians. All tailored Primary Care Reports from Ontario Health require ONE ID to authenticate users. Learn more about other services and resources available through ONE ID in the Overview of ONE ID Services.

MyPractice Primary Care Plus (with Screening Activity Report data) Registration:

If you are a physician:

  • Register for ONE ID via the College of Physicians and Surgeons of Ontario (CPSO) using the Registration Guide
    • eReport and MyPractice Primary Care Plus (with Screening Activity Report data) enrollment will be granted by Ontario Health within 1 business day of ONE ID registration.
  • Once enrolled, the Screening Activity Report (SAR) portion of the MyPractice Primary Care Plus (with Screening Activity Report data) will be available immediately.
  • Access your MyPractice Primary Care Physician Report by logging into eReport using your ONE ID and consenting to the notification to collect personal information.
    • The consent page will appear during your first eReport login, and is also available thereafter as a link within the MyPractice Primary Care Plus (with Screening Activity Report data).
    • After consenting, you will be able to review your MyPractice Primary Care Physician Report at the next refresh cycle.

If you are a delegate:

  • Physicians should send a registration request to oh-servicedesk@ontariohealth.ca on behalf of their delegate.
  • Once the request is received, your Local Registration Agent (LRA) or the Ontario Health Service Desk will book a registration appointment to set up your ONE ID account and enroll you to the MyPractice Primary Care Plus (with Screening Activity Report data).

Please note that physicians must first be registered with ONE ID before a delegate can be assigned to view their MyPractice Primary Care Plus (with Screening Activity Report data). Delegates may only view the embedded MyPractice Primary Care Physician Report if the physician has consented to the notification to collect personal information in eReport. SLZ SAR Registration:

If you are a Sioux Lookout Municipality and Sioux Lookout Zone Physician:

If you are a Sioux Lookout Municipality and Sioux Lookout Zone Nurse:

  • Send a registration request to oh-servicedesk@ontariohealth.ca and include your name and the CPSO number of the physician you support or your nurse registration number (ONA)
  • Once you have a ONEID, Send a request to ICCU@ontariohealth.ca to request access to view the SLZ SAR.

If you are a delegate:

Ontario Health has closely followed industry privacy and security standards in the development and operation of the MyPractice Primary Care Plus (including Screening Activity Report data).

Some information contained in the report is highly sensitive and contains personal health information. The disclosure of personal health information by Ontario Health to family physicians is permitted under the Personal Health Information Protection Act (PHIPA), 2004.

In handling data from First Nations, Inuit and Métis (FNIM) sources, we respect the First Nations Principles of Ownership, Control, Access and Possession (OCAP), Métis collective and self-determined data management and governance principles, and Inuit data governance principles.

How Physicians and Nurses Should Protect Patient Information

  • All users should remember that the report contains personal health information, which must be kept confidential at all times.
  • User names and passwords must never be shared.
  • The site must not be accessed from non-secure computer systems, such as a computer in a public library or cyber café.
  • Protecting the privacy of patient information is a shared responsibility. Once the report is downloaded, users are responsible for protecting the information on their systems and complying with PHIPA and their own organization’s privacy policies and procedures. This means, at a minimum, keeping computers up-to-date, running anti-virus software and scans, and using encryption.
