What is Ontario Health’s Primary Care Reporting Strategy?

Ontario Health is collaborating with primary care partners to develop a single consolidated report that meets the needs of the primary care sector. The work to create a tailored, consolidated, and comprehensive report is being achieved in multiple phases.

Phase 1

  • Develop foundational architecture to bring many data sources into a single report
  • Reduce the number of reports sent to primary care by creating a single report that streamlines access to the Screening Activity Report (SAR) and MyPractice Primary Care Physician Report
  • Transition to Microsoft PowerBI software, to improve data visualization capabilities
  • Expand MyPractice Primary Care Physician Report access to delegates
  • Simplify access through a single-sign-on button to the eReport portal within electronic medical record (EMR) platforms

Phase 2

  • Enhance user experience by re-designing the MyPractice Primary Care Plus (with Screening Activity Report data) to merge and align the embedded reports
  • Enhance available data by:
    • Merging in other primary care reports
    • Adding and/or modifying indicators and data stratifications

About the MyPractice Primary Care Plus (with Screening Activity Report data)

Why is the report only available online?

The MyPractice Primary Care Plus (with Screening Activity Report data) is only available through Ontario Health’s secure eReport portal to safeguard patient’s personal health information (PHI). This portal offers the convenience of being able to access the report electronically at any time in a user-friendly format.

Can I view the report on my smartphone or tablet?

While you are able to access the report on your smartphone or tablet, the report has not been optimized for use on such devices. Both the website and report have been developed for viewing on a laptop or desktop computer. For this reason, we do not recommend viewing the report on a mobile device.

Have content changes been made to the Screening Activity Report (SAR) and/or MyPractice Primary Care Physician Report with their transition into the MyPractice Primary Care Plus (with Screening Activity Report data)?

No, the contents of the SAR and MyPractice Primary Care Physician Report have not changed as part of their transition into the phase one iteration of the MyPractice Primary Care Plus (with Screening Activity Report data). Content changes and/or alignment may occur in future updates of the report.

Please note that data in the SAR reflects a physician’s Patient Enrollment Model rostered patients whereas the MyPractice Primary Care Physician Report is based on both rostered and non-rostered patients. For non-rostered patients, a virtual rostering methodology was applied in which patients are attributed to the family physician who have billed or ‘shadow’ billed the largest dollar amount of core primary care services for the patients in the previous two years. Alignment of methodology across other indicators may be incorporated into future releases of the MyPractice Primary Care Plus (with Screening Activity Report data).

Why are other Ontario Health reports, like the Sioux Lookout and Zone Screening Activity Report (SLZ SAR), not included in the MyPractice Primary Care Plus (with Screening Activity Report data)? Are there plans to enhance the report?

The work to develop a tailored, consolidated, and comprehensive report for quality improvement in primary care, will be achieved in multiple phases (please see What is Ontario Health’s Primary Care Reporting Strategy). In phase 1, foundational architecture was developed to bring in data from many sources. As a result of this, the first iteration of the MyPractice Primary Care Plus (with Screening Activity Report data) streamlines access to the Screening Activity Report (SAR) and MyPractice Primary Care Physician Report. The SLZ SAR was not included in phase 1, due to technical complexities of a postal code based report. In phase 2 and 3 of this work, the MyPractice Primary Care Plus (with Screening Activity Report data) will continue to evolve and future updates may include the addition of other reports (like the SLZ SAR), indicators and report audiences.

How do I action the data within the report to support quality improvement?

To support quality improvement for your MyPractice Primary Care Physician Report indicators, please review the Quality Improvement pages. You may find these at the top of the eReport page under the MyPractice QI Suggestions and References heading or by clicking the hyperlinked text on the MyPractice indicator pages. You can also access many resources to support using data for quality improvement in the Primary Care Quality Improvement Hub.

For the Screening Activity Report, please ensure that you review enrolled patients who are flagged as Action Required (highlighted in red), Due for Screening or Physician Review Required (both highlighted in yellow), and follow-up based on screening program/results report recommendations. You can find these patients by navigating to the Patient Summary or indicator-specific Patient View tabs. Alternatively, you may navigate to these pages by right-clicking on the table in the SAR Home tab and selecting Drill Through.

What is a delegate and what are the advantages of authorizing a delegate?

A delegate is a member of your primary care team (e.g., a nurse, office manager or clinic staff member) who is authorized to access your MyPractice Primary Care Plus (with Screening Activity Report data). A delegate is set up with their own unique ONE ID account and is given permission from a physician to view the report. By authorizing delegates, the tasks involved in quality improvement can be shared among team members. How do I view and print my MyPractice Primary Care Plus (with Screening Activity Report data)?

You can view the report from the eReport website. Select MyPractice PC Plus SAR from the selection bar and the report will load to the Landing Page.

From the Landing Page, there are buttons to take you to the Screening Activity Report (SAR) and MyPractice Primary Care Physician Report homepages. You may also use the Navigation tab to jump between different report pages.

To print your reports, use your computer’s built-in printing tool. On Windows computers, use CTRL + P to print the current screen. On Apple computers, use ⌘ + P to print the current screen.

If there is one visualization or dataset you would like to print, you can do this by taking a screenshot of the relevant visualization (Windows: CTRL + window key+S; Apple: ⌘+SHIFT+4).

To export the data and recreate the visualization in Excel. You may do this by hovering over the visualization, selecting the three dots that appear at the top and selecting “Export data”.

You are responsible for managing any reports or documents printed or otherwise extracted from the eReport portal in accordance with Personal Health Information Protection Act (PHIPA) and your organizations policies and procedures.
