Acute Leukemia Shared-Care Program in the Greater Toronto Area

The Acute Leukemia Shared-Care Program makes sure adults with acute leukemia get timely access to high-quality, coordinated services as close to home as possible. Where appropriate, patients may receive some of their care at Princess Margaret Cancer Centre and some at a partner cancer centre closer to home.

Goals

  • Maintain the provision of high quality care
  • Reducing the burden on the Princess Margaret Cancer Centre’s resources
  • Reducing the travel pressures experienced by patients and their families

 

The program is now available at select centres in the Greater Toronto Area (GTA). We will continue to work with regional partners to expand the Shared-Care Program to more centres. We will use several performance evaluation strategies to measure access and coordination of care provided in the program.

 

Program Background

Historically, the care of adults with acute leukemia in the GTA has been highly centralized. Most people received treatment and follow-up care at Princess Margaret Cancer Centre. This resulted in significant resource pressures at the centre and traveling pressures for patients who live far away.

 

In 2011, we created the Acute Leukemia Steering Committee to look at how Ontario might better meet the demand for leukemia services in the GTA. The result was a recommendations report – Adult Acute Leukemia Services Plan for the Greater Toronto Area.

 

The report recommends:

  • ensuring adequate access to, capacity for and sustainability of acute leukemia services throughout the patients’ treatment journey
  • advancing the quality and safety of leukemia services
  • coordinating and overseeing acute leukemia services
